SERVICES:
An individual must be terminally ill and receiving hospice services to be eligible for assistance from CCHF. They must also be able to demonstrate a financial need. The use of CCHF funds must greatly improve the quality of life for the patient or the patient’s family.
CCHF routinely provides support for the following things:
Mortgage, Rent, Utility Bill, Food, Phone, Lifeline, Environmental and Emotional Supports - fan, heater, air conditioner, airline tickets to bring a daughter home to care for her parent (daughter must also be in financial need), clothing, food, a refrigerator for a family with small children who had been storing their food in coolers, etc…
IMPORTANT:
CCHF Funds are limited to assist hospice patients and their caregivers who demonstrate a financial need. An actual bill showing vendor name and address & account information MUST accompany all requests. Unfortunately, CCHF CANNOT cover funeral or burial related expenses, car or auto insurance payments, home health aids, medications, or medical equipment. Payments are NEVER made to patient or patient/family/caregivers – ONLY to a vendor. Additionally, Board approval is mandatory if request falls outside of the traditional guidelines or the request exceeds $300. Verification of need and current financial information for the entire household MUST be included in the request. Please call our office for an application or for additional information. 302-368-8944
