Checks, Grants & Compliance…
I’m responsible for writing checks once an approval has been given for a request. If you’re looking for information about a check (date sent, check number, etc...) give me a call.
ALL REQUESTS
Must be submitted by a hospice professional and approved in advance. All requests must be accompanied by a readable bill or invoice that includes the mailing address of the creditor and the name and account numbers for the debtor. Sometime this information is different than that of the patient and we want to make sure the check is properly credited to the correct account.

LIFELINES
Lifelines always need to be approved PRIOR to securing a service. The local Social Worker is expected to insure the return of the LIFELINE UNIT once a patient has passed away. This will prevent CCHF from having to pay a $500 replacement fee.

How does the family know I have made a contribution?
Contributors will receive a thank you letter within 48 hours and families will receive and acknowledgment letter indicating names and addresses of contributors several weeks after CCHF receives donations in memory of their loved one. Typically we send this information to the person who is listed as the next of kin. If you would like another family member notified - please include that in a note with your donation.
Why does it take so long?
CCHF collects a group of names as opposed to sending out multiple mailings once we receive checks from a local office. Sometimes the local office will wait until they have several checks before mailing them to CCHF. If a family calls with a concern always feel free to call or email me at 302-368-8944.
